As Pope Alexander (Jeremy Irons) fights for his life after being poisoned, the Cardinals jockey for position to take over the Papacy. Cesare (Francois Arnaud) and Micheletto (Sean Harris) trace the assassination plot back to its source, but Della Rovere (Colm Feore) is ready to make his move -- and his is not the only conspiracy. Catherina Sforza (Gina McKee) has dispatched her own personal assassin Rufio (Thure Lindhardt) to dethrone the Borgia family. However, thanks to Lucrezia’s quick thinking (Holliday Grainger), the family’s grip on power may not be lost.

As the Pope has stopped writhing in agony and hangs almost lifeless on the verge of death after having been poisoned by his taster, the Vatican's resident physician is hastily awaken in the middle of the night and led to the hall that mere moments ago played host to Lucrezia's betrothal celebration. As guests and by-standers are being forced out of the hall and the Borgia family gathers around the dying Pope eagerly awaiting the physician's diagnosis, he proclaims that nothing can be done to counter the effects of cantarella. But neither Cesare nor Lucrezia will tolerate that and the Pope's daughter, thinking on her feet, immediately applies an unorthodox and bizarrely violent remedy that prolongs the Pope's survival, even in such a suspended state between life and death. As Alexander VI is carried to his chambers under the escort of guards, Vanozza, Lucrezia, Giulia and the cardinals that already start whispering premature plans to replace the Pope, Cesare and Micheletto leave the Vatican when the latter uncovers a clue hidden within the poisoner Antonello's cell that could lead them to the culprit of the aggression against Alexander VI.

With the Pope's life still in danger and the cardinals dreaming of his demise, Cesare and Micheletto storm the Dominican abbey in Rome and massacre everyone only to discover that the monks protected Borgia nemesis Della Rovere and subsequently helped him escape in time. The cardinal may have evaded Cesare's grasp but his endgame is clear: he returns to the Vatican in order to maneuver into position for an upcoming conclave, mistakenly thinking that the Pope's death is a foregone conclusion. Cesare swiftly comes back and near assaults the treacherous Della Rovere before getting reined in by his mother Vanozza who is already contemplating a future without the lavish protection of the Pope of Rome. The entire Borgia clan is temporarily barricaded inside the Vatican to hold vigil at the Pope's side but Giovanni is carried back to Vanozza's villa by Micheletto and his men for his own tranquility.

The next morning Cesare approaches Cardinal Sforza and demands guidance in the matter of the convoluted consistory politics. Sforza obliges but is simultaneously faced with a dilemma, presented by his mighty cousin Caterina Sforza through her own enforcer Rufio: Ascanio Sforza learns that the entire Borgia family is soon to be exterminated and he can either aid in the plot by assassinating the Pope himself hoping to succeed him on Saint Peter's throne or face the wrath of Alexander VI when he wakes up to find his kin obliterated by Sforza agents.

Meanwhile, the Pope's unexpected recovery from his coma forces Della Rovere to flee but his attempt is futile and he soon gets arrested by Cesare. Gathering his family around him and still bemoaning the death of Juan, Rodrigo in a state of shock and fear, reveals that his faith is terribly shaken and plots with Cesare to frame someone for the murder of his middle son or else Italy will soon suspect Cesare's hand in the deed.

The imprisoned Della Rovere tries to entice Cesare into releasing him with promise of help when Rodrigo eventually dies but Cesare coldly refuses and departs. Cardinal Giovanni Battista Orsini bribes and kills Della Rovere's captors and sets him free, providing him with the clothes of a peasant and arranging to smuggle him out of Rome, that is under a lockdown imposed by Cardinal Sforza, hidden in a cart carrying dead bodies.

Ascanio Sforza is finally persuaded to help in Caterina's plot, as Rufio's preparations to infiltrate Vanozza's household are proceeding with speed. Supplied with a lethal artifact, Ascanio gets caught by Cesare seconds before he can complete his mission of murdering the sleeping Alexander VI and is forced to flip on his cousin in order to assert his innocence. Cesare, informed of the plot to wipe out his family, thwarts Rufio's subterfuge with the help of Micheletto and the two men kill the assailants and the maidservant that functioned as Rufio's spy within the villa.

Caterina Sforza is furious when she learns that Ascanio betrayed her and warned the Borgias, effectively rescuing them and decides to look for allies outside the College of Cardinals. She dispatches Rufio to marshal under her banner all the disenfranchised houses of central Italy that hold a grudge against the Borgia papacy, in order to build a larger power base before moving against the family again.

The still recovering Rodrigo is livid to be informed by Cesare that Caterina Sforza unsuccessfully targeted his family and he declares war on everyone that antagonized him a
d his office.
